Consistent crashes under OS X- screen freezes, no control, no automatic reboot

Hey,

So I've been having fairly consistent crashes under OS X, where the screen completely freezes, leaving me with no control.  The only option at that point seems to be to cause a kernel panic by unplugging the akitio/holding the power button to turn it off and reset.  This doesn't seem to happen at any particular time.  Any ideas? (setup and crash log below).

Hi, first guess would be the powered riser, can you test it with another one?
Or even better eliminate the riser and put the GPU drectly into the AKiTiO and power it with a modded barrel plug?!

I've already had one broken AKITIO, I'm trying not to unscrew/mod anything with the box in case this one dies too.  Ill order another powered riser from amazon though, if thats a likely failure point.

I have a Late 2013 27" iMac with TB1, Akitio, and Titan Z.

 

But honestly when I ended up modding the Akitio and getting a barrel plug mod... it was the best thing I ever done. Now the Akitio/GPU setup starts and shuts down with my computer perfectly. Where as I use to have to power up the PSU & Computer seperatley... and the fan on my Titan would spin heavily when I shut down my computer.
